Release Agreement and Photograph/Video Permission for Adults

I, , wish to participate in education programs provided through the Monte L. Bean Museum. I have been advised of and/or recognize the risks inherent with my participation in this BYU sponsored program. I understand that I may be traveling in University vehicles to different educational locations. I assume full responsibility for all injuries that may arise from participation therein and for all risks associated with my physical or emotional limitations. I unconditionally release BYU and its employees from any and all liability or claims that might result from my participation in this program, unless injury or damage is primarily the direct result of negligence on the part of BYU or any of its employees and not caused in part by my own negligence.

In the event of any illness or injury while participating in this program, I give permission for the museum to transport me to a local medical facility to receive treatment; and I guarantee payment of all expenses incurred for such transportation and treatment.

For the benefit of the programs administered by the Monte L. Bean Life Science Museum, I do hereby grant to the Bean Museum permission to photograph, videotape, and audiotape me. I further grant to the Bean Museum permission to publish and/or publicly exhibit the photographs, videos, and soundtracks in any lawful and legitimate manner for the purposes of program development, education and/or promotion.
